Essential Information
Tdap information:
If your child's last immunization that contained tentanus was given as least five year ago, your child must have a booster shot before the start of school on August 18, 2015. This shot may be listed as T, Td, Dtap, and/or Tdap. Check with your child's health care provider or a local health department if you have questions.
Shots may be obtained from your doctor, military clinics, or the health department.  Children ages ten and up may also receive the vaccine at one the Teen Health Centers. Local health department and the Teen Health Centers offer the vaccination to rising 6th graders free of charge, but appointment must be made in advance. Be sure to take a copy of your child's immunization record to the appointment.  Health Department and Teen Health Center Information is as follows:

IF DOCUMENTATION THAT YOUR CHILD HAS RECEIVED THIS IMMUNIZATION IS NOT PROVIDED TO THE SCHOOL, BEFORE SCHOOL STARTS , YOUR CHILD WILL NOT BE ALLOWED TO ATTEND SCHOOOL UNTIL SUCH PROOF IS PROVIDED.

When checking out your student, please be prepared to present a photo I.D.  Woodrow Wilson will not release a student to anyone other than parent or legal guardian.

Annual Middle & High School Sports Physicals
Hosted by Northside High School
Saturday, May 21, 2011
$15.00 per Student

The physical will include:    - Review of Medical History, Height & Weight, Blood Pressure, Urinalysis,
Range of Motion Evaluation, Vision Test, Assessment of Mouth, Heart,
Lungs & Spleen, and Hernia Assessment for Male Athletes.


This event is offered to students entering grades 7 - 12 in the 2011-2012 acacemic year.
This physical qualifies students for sports participation 2/21/11 - 5/31/12.
